HIPAA Policy

Effective Date: 3/1/2024

Introduction Accelerate (“we,” “our,” “us”) is committed to protecting the privacy and confidentiality of our patients’ health information. This Privacy Policy outlines how we handle Protected Health Information (PHI), your rights regarding your health information, and how you can contact us for further information. We comply with the Health Insurance Portability and Accountability Act (HIPAA) and related regulations to ensure your information is protected.

How We Treat Protected Health Information (PHI)

We collect, use, and disclose PHI only as permitted by HIPAA and applicable laws. PHI refers to any information about your health, healthcare services, or payment for healthcare services that can identify you.

  1. Uses and Disclosures of PHI:
    • For Treatment: To provide, coordinate, or manage your healthcare and related services.
    • For Payment: To obtain payment for healthcare services provided to you.
    • For Healthcare Operations: To evaluate and improve the quality of care we provide.
    • As Required by Law: When mandated by legal or regulatory requirements.
  2. Safeguards for PHI:
    • We implement administrative, technical, and physical safeguards to protect your PHI from unauthorized access, use, or disclosure.
    • Only authorized personnel have access to PHI on a need-to-know basis.
  3. Third-Party Disclosures:
    • We may share PHI with business associates (e.g., billing companies, telehealth platforms) who assist us in providing services. These associates are contractually obligated to protect your information.

Your Rights Regarding Protected Health Information

Under HIPAA, you have the following rights concerning your PHI:

  1. Right to Access:
    • You may request a copy of your medical records in electronic or paper format. We will provide these records within the timeframe required by law.
  2. Right to Amend:
    • If you believe your health information is incorrect or incomplete, you can request an amendment. We will review your request and respond promptly.
  3. Right to Restrict Disclosures:
    • You may request limitations on how we use or share your PHI. While we will consider your request, we are not obligated to agree in all cases.
  4. Right to Confidential Communications:
    • You can request that we communicate with you in a specific manner or at a specific location (e.g., sending correspondence to a P.O. Box).
  5. Right to an Accounting of Disclosures:
    • You have the right to receive a record of certain disclosures of your PHI made outside of treatment, payment, or healthcare operations.
  6. Right to File a Complaint:
    • If you believe your privacy rights have been violated, you can file a complaint with us or with the U.S. Department of Health and Human Services. Filing a complaint will not affect your care.
